SUMMARY/JOB PURPOSE:The Youth Mental Health Case Manager provides
evidence-based services to children and their families in
maintaining or improving their ability to function in the community
and work toward their recovery goals. This position assists youth
with mental illness and/or behavioral issue and their families
access medical, social, educational and other appropriate services.
This position encourages youth to live their best lives through
support, resources, skills training, parent training, safety
planning, crisis intervention, and many other services. The Youth
Mental Health Case Manager works in collaboration with other
agencies within the community and state to link youth and their
families to other needed services. This position provides
monitoring, assessment, service planning and coordination, and
skills training and development.This position works independently,
under limited supervision, reporting major activities through
periodic meetings.EDUCATION, EXPERIENCE, OTHER
QUALIFICATIONS:Education Required: A Bachelor's degree from an
accredited college or university with a major in psychology, social
work, medicine, nursing, rehabilitation, counseling, sociology,
human growth and development, physician assistance, gerontology,
special education, educational psychology, early childhood
education or early childhood intervention or a bachelor's degree
with at least 30 hours of coursework in the previous
fields.Experience Required: Previous experience in the field of
mental health or social services preferred.Registration,
Certification, Licensure or other Qualifications Required:
- Must maintain a valid Texas Driver's license, auto liability
insurance and a driving record acceptable to PermiaCare's insurance
requirements.
- Required to pass criminal history and background checks as well
as pre-employment drug screen.
- Must obtain QMHP certification within 6 months of
hire.ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Complete and update Recovery Plans prior to due date.
- Update Uniform Assessments Forms prior to due date.
- Develop appropriate Crisis/Safety Plans.
- Provide skills training with youth face to face.
- Provide parent skills training face to face.
- Provide routine case management, monitoring and observation
with youth and their families.
- Provide Wraparound Facilitator services as regulated by the
State, if certified.
- Ensure service plans incorporate the findings of the intake
assessment, uniform assessment and individual choices of the youth
as well as clearly identify client needs, goals and strategies for
achieving goals.
- Ensure recovery plan reviews clearly show progress or lack of
progress for the last required period.
- Learn and apply the Medicaid covered services for this
position, the proper application of these services and the codes
used to describe these services.
- Assess individual needs.
- Develop, implement and review recovery plans and work with
other community resources in meeting and achieving the individual
and family service needs.
- Provide clerical services, service coordination, and outreach
and engagement activities to caseload including completion of needs
assessment, individualized recovery planning, referral to
appropriate services, monitoring of needs, and community
engagement.
- Provide crisis services and problem solving assistance to
caseload as well as families.
- Provide treatment planning per program guidelines.
- Address problems such as risk of harm, crisis intervention and
resolution, substance abuse, family/relationship differences,
limited community involvement, individual's understanding of
illness, medication non-compliance, limited coping skills.
- Complete Uniform Assessment and Wraparound plans.
- Develop and review recovery plans.
- Perform follow up and monitor status of patient care.
- Provide case management and rehab services, including
assessment, service plan development, coordinating services,
monitoring advocacy and family education.
- Provide YES services for caseload of seriously emotionally
disturbed youth.
- Track progress toward goals and objectives and adjust
wraparound plans as needed.
- Attend peer review meetings as required.
- Obtain client vitals and enter into required documentation, as
required.
- Meet unit performance measures or targets.
- Maintain assigned caseload.
- Coordinate appropriate services to designated caseload.
- Enter accurate and appropriate documentation of services within
timeframe required.
- Maintain confidentiality of sensitive records and treatment
information, client files and protected health information in
compliance with HIPAA, laws, rules and regulations, and established
procedures.
- Maintain regular and reliable physical on-site attendance.
Regular attendance, dependability, and promptness are required for
the scheduled work day 100% of the time, to ensure consistency and
completeness of program's processes.
- Comply with the Abuse, Neglect, and Exploitation policy and
reporting requirements.
- Adhere to the Code of Conduct and Standards of Behavior policy
requirements.
- Establish and maintain effective work relationships with
individuals served and their families, supervisors, co-workers and
visitors by demonstrating cooperative, courteous and respectful
behavior at all times.
- Communicate regularly with supervisor.
- Open and process mail/email in a timely manner.
- Answer phone, collect phone messages and respond to requests
timely and accurately.
- Maintain safe and clean working environment by complying with
procedures, rules and regulations.
- Perform all work functions and interactions using a trauma
informed approach.
- Display professionalism when representing PermiaCare and the
program in the community.
- Maintain compliance with legal requirements and company
policies and procedures.
- Maintain valid and current driver's license, auto insurance,
acceptable driving record and reliable transportation at all times.
Driving may be required for this position.
- Complete all training as assigned prior to due date.
