- ALL travelers must completed, regardless of TB result.

- Physical Exam required within 1 year of start, does not need to
be updated annually.
- N95 Fit Test required within 1 year of start. Facility will not
accept PAPR, does not need to be renewed annually.
- Negative PPD, QGold, or TSpot required within 1 year of start.
Positive result requires a Chest X-Ray within 5 years and annual
TBQ.
- Flu Vaccination or Agency Declination required October 1 -
March 31. If candidate declines, they may be asked to wear a mask
in certain areas.
- Preferred, but no longer required. Collect if the traveler is
able to provide.
- Background Check is required. Must be renewed annually.
- Lifetime Criminal Background Check required within 30 days of
start. Must run complete potential criminal history (Lifetime) for
all locations lived and worked, including aliases. Updated
Annually.
- EPLS/GSA/SAM required within 30 days of start.
- HHS/OIG required within 30 days of start.
- 2 References required within 3 years of start.
- Copy of Resume/Application required.
- Skills Checklist 1 required. Update annually.
- Texas Department of Public Safety Background Check required
within 30 days prior to start date.
- Texas Department of Aging & Disability Services Check EMR -
Employee Misconduct Record required within 30 days prior to start
date.
